BULK BILLING

Bulk billing is currently available to concession card holders and children under the age of 16, if consent is obtained as per Medicare guidelines.  Certain services may involve extra charges and will not be bulk billed.  These services include lesion removals, minor surgical procedures, treatment of fractures and other procedures.

MISSED APPOINTMENT FEE

A $45.00 Missed Appointment fee will be payable if you do not attend your appointment or fail to cancel your appointment at least 2 hours prior to the appointment time. 

REPEAT SCRIPTS & REFERRALS

Patients are asked to make an appointment for repeat scripts or referrals.

Copies of lost referrals will incur a $30 admin fee.

TRANSFER OF MEDICAL HISTORY

Leongatha Healthcare will provide a Health Summary to your new GP at no cost. Fees will be incurred for any additional information required. All outstanding accounts are to be paid prior to transfer of medical records being completed.
